2019 Theme Issues - Anyone Have Any Ideas?

Hi,

We have a few issues with the 2019 theme on a fresh hostbill installation - the theme is not modified, only the header and footer have been changed.

We have created a list which is attached. Does anyone have any issues on how to fix these issues?
